Exercise Therapy for Chronic Low Back Pain: What the Largest Review to Date Found

A major Cochrane review of 249 clinical trials found that exercise therapy is likely to reduce pain and improve day-to-day function in people with chronic low back pain, compared with no treatment or usual care.

Chronic low back pain — pain that lasts longer than about three months — is one of the leading causes of disability worldwide. For decades, researchers have studied whether structured exercise actually helps, and this 2021 Cochrane review is the most comprehensive summary of that research to date. Cochrane reviews are widely regarded as one of the most rigorous forms of medical evidence because they systematically gather and appraise every relevant trial.

The authors, led by Dr. Jill Hayden, analyzed 249 randomized controlled trials involving tens of thousands of adults with chronic low back pain. The trials tested many forms of exercise — general conditioning, strengthening, stretching, core stability, and more — delivered in clinics, gyms, and home programs.

The headline result: exercise therapy probably reduces pain and improves physical function compared with doing nothing, usual medical care, or placebo-style comparisons. The average benefit at the earliest follow-up was around 7 points out of 100 for pain and around 2.5 points out of 100 for function. Those numbers may sound small, but they are averages across many different programs and people — and exercise also tended to perform as well as or better than other common conservative treatments studied. Importantly, the review found that adverse events from supervised exercise were uncommon, and the authors concluded exercise is a safe option for this population.
