Can Exercise Help Chronic Pain? An Umbrella Review of the Evidence

An overview of Cochrane reviews across many chronic pain conditions found that exercise and physical activity tend to improve pain severity and physical function, with few reported harms.

Chronic pain — pain that outlasts normal healing time, generally beyond about twelve weeks — affects a large share of adults and can erode sleep, mood, work, and quality of life. A common worry for people living with it is whether exercise will make things worse. This publication set out to answer that at the broadest possible level: it is an 'overview of reviews,' meaning the authors gathered existing Cochrane systematic reviews on exercise for different chronic pain conditions and summarized what they collectively show.

The conditions covered ranged across common sources of persistent pain, and the exercise interventions were just as varied: aerobic activity, strength training, flexibility work, water-based programs, and mind-body forms of movement, delivered at different intensities and durations.

The overall picture was cautiously positive. Across the included reviews, physical activity and exercise tended to reduce pain severity and improve physical function, and some reviews found improvements in quality of life as well.
